Black Swan Health employs a team of friendly, in-house dietitians who can provide nutrition advice tailored to your needs.
Our dietitians can help with a large range of conditions including;
- Weight management
- Sports nutrition
- Coeliac disease
- Food intolerance/ allergy
- Irritable bowel syndrome
In addition to our one-on-one session we also offer all clients free access to our Health and Wellness Group, a five-week healthy lifestyle program which is run at venues throughout the north metropolitan area. Click here to find out more.

How can I access your services?
Just call our Health and Wellness team on 9201 0044 to arrange an appointment. You do not require a GP referral to access our services.
Where are the appointments available?
Appointments are offered in easily accessible locations in Joondalup, Osborne Park and Midland.
How much do these appointments cost?
The cost of the appointment varies depending on your situation. We can confirm the cost when you contact us.
- Clients referred by their GP may be eligible for a Medicare rebate.
- Clients with private health cover may be eligible for a rebate from their private health provider.
- DVA Health Cards (Gold and White) are accepted as payment upon GP referral.
We are a not-for-profit organisation so any payments we receive are reinvested in order to deliver services that improve the health and mental wellbeing for individuals in our local community.
We have HICAPS available at all sites and so are able to process rebates immediately.

For General Practitioners
It is very easy to refer your clients to our dietetics services. Just download and complete the referral form below. Please ensure that you attach a copy of the GP Management Plan and Team Care Arrangement.
Clients will be asked to pay the full cost of the consultation and a Medicare rebate will be refunded into their bank account via HICAPS. Concession card holders will be bulk billed.
