Referral pathways
- Better Access initiative: GP referrals with a Mental Health Care Plan are eligible for a Medicare rebate. Completed referral forms and MHCPs can be emailed or faxed.
- Workers Compensation and DVA: Please email referrals to psychology@blackswanhealth.com.au.
- Self Referral: Clients can self refer and those with private health cover may be eligible for a rebate.
Assessment services now available for adults
Our psychological assessment team have appointments available at our Osborne Park and Joondalup clinics. The current wait time is approximately 4 weeks. A referral is not required to access our assessment services.

- Comprehensive assessment (full academic assessment and ADHD)
- Ages 6 to 16
- Full academic assessment (WISC-IV and WIAT-4)
- Ages 6 to 16
- Cognitive assessment (WISC-IV)
- Ages 6 to 16
- Educational assessment (WIAT-4)
- Ages 4+ including adults
- ADHD assessment
- Ages 3+ including adults
- Autism Spectrum Disorder assessment
- Ages 3+ including adults
Clients with private health insurance may be eligible for a rebate. NDIS clients may be eligible to pay for an assessment as part of their funding package. Clients can complete the enquiry form here and one of our helpful team members will make contact to discuss individual needs, our testing structure and what to expect during the assessment process.
Eating Disorder Treatment Service
Our dedicated Eating Disorder Treatment Service uses best practice approaches based on the latest empirical evidence to help clients with a range of disordered eating behaviours and disorders.
Our highly skilled Clinical Psychologists and Accredited Practising Dietitians work together to deliver this specialised service at our Midland, Osborne Park and Joondalup locations.
