Type 2 Diabetes: Group Programs
Time 2 Manage Diabetes
Black Swan Health’s Time 2 Manage Diabetes is an eight week group exercise program run by an Accredited Exercise Physiologist. Clients attend an initial assessment during which a comprehensive medical history will be taken, baseline measures are recorded and exercise goals are established and agreed on.
Clients attend eight group exercise sessions at one of our gyms in either Osborne Park or Midland. The group sizes are small and each session lasts one hour. Our Accredited Exercise Physiologist tailors exercises based on each client’s individual needs.
To be eligible
Clients need to be aged over 18 and have a confirmed diagnosis of Type 2 Diabetes.
The program is delivered at our Osborne Park and Midland gyms.
GPs can refer to the program using a Referral form for Group Allied Health Services under Medicare for patients with type 2 diabetes which can be downloaded here. Please ensure that you attach a copy of the GP Management Plan with the referral
Please note that these group services are in addition to the five individual allied health services available to eligible patients under a Team Care Arrangement.
- Clients referred by their GP using a Referral form for Group Allied Health Services under Medicare for patients with type 2 diabetes can access the program free of charge.
- Clients who self-refer will be charged an initial fee of $65 and $15 per session.
