Continuous Glucose Monitoring

About the Service 

Continuous glucose monitoring involves wearing a small glucose sensor which measures glucose levels every five minutes. It is normally worn for a six day period. After this the data is uploaded and plotted onto graphs, enabling identification of glucose patterns.

Continuous glucose monitoring can be offered to clients on multiple daily injections, including people with Type 1 and Type 2 Diabetes. It is of particular benefit to clients who are experiencing fluctuations in their blood glucose levels (especially overnight) and who struggle to match their insulin with their carbohydrate intake. It can provide an insight into causes of variability and help the client and their diabetes team see the bigger picture of what is happening, and in turn, help guide adjustments.

Process

Clients attend an appointment with a Credentialled Diabetes Educator for sensor insertion and device training. They then attend a second appointment, where the transmitter will be returned and the data will be uploaded. An individualised report will be produced showing glucose patterns which can be reviewed with the client. The report can be sent to the client’s GP.

Referrals

Clients can self-refer or be referred by their GP.

How much does this service cost?

The cost of the service varies depends on the rebates that apply. Clients referred by their GP under a Team Care Arrangement may be eligible for a Medicare rebate. There is a cost for the sensor but we will loan the transmitter.

For us to be able to offer your client a Medicare rebate please send us a copy of the GP Management Plan and a Team Care Arrangement with two occasions of service allocated to a CDE.
