Type 2 Diabetes: Individual Programs

About the program

Our Credentialled Diabetes Educators and Accredited Practising Dietitians use a client-centred approach in the provision of diabetes self-management education.

To be eligible

To be eligible clients need to be aged over 18 and have a confirmed diagnosis of type 2 diabetes.

Type 2 diabetes program: what clients can expect

Clients receive an initial assessment that covers the following:

  • Guidance on managing their diabetes
  • Advice about healthy eating
  • Identification of specific health needs and;
  • Development of individualised treatment plans and goals.

Bookings and referrals

Call our Health and Wellness team on 1300 820 398 (option 1 and 3) to arrange an appointment. A GP referral is not required to access our services. Telehealth and in person appointments are available

Locations

Individual appointments can be organised at our Joondalup, Osborne Park and Midland clinics.

Pricing

For private diabetes education appointments, we charge $98 for an initial appointment and $80 for review appointments.

  • Clients referred by their GP under a Team Care Arrangement may be eligible for a Medicare rebate.
  • Clients with private health cover may be eligible for a rebate from their private health provider.
    • DVA Health Cards (Gold and White) are accepted with no out of pocket expenses if referred by their GP.

For General Practitioners

Referrals to our services are simple; just download and complete the referral form. Please ensure that you attached a copy of the GP Management Plan and Team Care Arrangement. Clients will be asked to pay the full cost of the consultation and a Medicare rebate will be refunded into their bank account via HICAPS if applicable.
