About the program

Our Credentialled Diabetes Educators and Accredited Practising Dietitians use a client-centred approach in the provision of diabetes self-management education.

To be eligible

To be eligible clients need to be aged over 18 and have a confirmed diagnosis of type 2 diabetes.

Type 2 diabetes program: what clients can expect

Clients receive an initial assessment that covers the following:

  • guidance on managing their diabetes
  • advice about healthy eating
  • identification of specific health needs
  • development of individualised treatment plans and goals

One referral lasts 12 months and clients are offered reviews at three, six and 12 month intervals. At each review, clients are monitored and evaluated. They may be referred to other healthy lifestyle support services, including one of our diabetes group programs.

Individual appointments can be organised at our Midland, Mirrabooka, Osborne Park, Joondalup and Fremantle clinics.

Program costs

The program is free. Clients will not be charged a gap fee.

Referral process

Referrals are accepted from GPs and other health professionals. Referral forms can be downloaded in PDF or Best Practice format here.

We request that a client’s referral is accompanied by a Referral form for Individual Allied Health Services under Medicare for patients with a chronic medical condition and complex care needs for eligible clients.
