We offer 3 group programs for clients with type 2 diabetes:
Please note that these group services are in addition to the five individual allied health services available to eligible patients.
DESMOND (Diabetes Education and Self-Management for Ongoing and Newly Diagnosed) is an evidence-based education course for people with type 2 diabetes.
The interactive program is built around group activities and includes topics such as healthy food choices, physical activity and medication management.
DESMOND is a six-hour program delivered as a full day session. We have facilitators trained by Diabetes WA to deliver this program.
We deliver DESMOND from Joondalup, Midland, Osborne Park and Fremantle.
Time 2 Manage Diabetes
Time 2 Manage Diabetes is a five-week group education and exercise program. The group meets for 2 hours each week.
The first hour is an interactive education session with a Credentialled Diabetes Educator or Accredited Practising Dietitian. Education topics include:
- Basics of diabetes including blood glucose monitoring and the management of hypoglycaemia and hyperglycaemia
- Annual cycle of care
- Healthy eating for diabetes including carbohydrates and glycaemic index, fats, label reading, portion control
- Weight management
The second hour is a supervised group exercise session, tailored to individual requirements, with one of our physiotherapists.
We deliver this program from Osborne Park, Joondalup and Midland.
Time 2 Manage Diabetes Evening Program
Time 2 Manage Diabetes Evening Program has been developed for people who cannot attend one of our day time groups.
The group meets for three weekly sessions from 6-8pm and is facilitated by a Credentialed Diabetes Educator and Accredited Practising Dietitian. This program includes education on blood glucose monitoring, annual cycle of care and healthy eating for diabetes. It does not include an exercise group.
We offer this program from Osborne Park and Joondalup.
To be eligible for type 2 group programs
Clients need to be aged over 18 and have a confirmed diagnosis of type 2 diabetes.
Type 2 diabetes group program cost
There are no fees to attend any of our type 2 group programs.
Referral process for type 2 group programs
GP can refer to the program using a Referral form for Group Allied Health Services under Medicare for patients with type 2 diabetes which can be downloaded here.
Please ensure that you attach a copy of the GP Management Plan with the referral.
