Psychosocial Supports Program

Black Swan Health’s Psychosocial Supports Program (formerly Partner in Recovery – PIR) assists people with severe, persistent mental illness with unmet needs, as well as their carers and families.

Our staff assist individuals to make an application for an NDIS package, while at the same time working with multiple sectors, services and supports to assist with the improvement of our clients’ improved health, wellbeing and recovery.

The program helps people who:
  • Have long-term, severe mental health issues
  • Have needs that can’t be addressed by just one support service
  • Are struggling to get the help they need from other places

Program objectives

The program aims to link clients with services that offer supports in the following areas:

  • Social skills and connections, including family connections
  • Day to day living skills
  • Financial management and budgeting
  • Finding and maintaining a home
  • Vocational skills and goals;
  • Maintaining physical wellbeing, including exercise; and
  • Building broader life skills including confidence and resilience.

What we do

  • Assist clients to access an NDIS plan
  • Develop plans to work towards improved health, wellness and recovery
  • Find the right places to get help
  • Coordinate different services that can meet clients’ needs and;
  • Help find solutions to practical problems (e.g. housing and health needs).

Referral process

Self-referral

People can refer themselves into the program by completing the Psychosocial Supports referral form and submitting to Black Swan Health via fax (9201 0033) or email psychosocial@blackswanhealth.com.au

Alternatively, they can self refer over the phone by calling (08) 9201 0044.

Professional referral

GPs, allied health professionals, community-based agencies and educational institutions can all refer clients to Psychosocial Supports using the referral form available here.

Please note: the client must consent to being referred into the Program.
