About the Service
Black Swan Health offers respiratory services to educate and support respiratory clients to effectively manage their condition and improve their lifestyle. These services aim to prevent or reduce exacerbation and hospitalisations through effective condition management and enable clients to achieve and maintain a maximum level of independence and functioning at home and in the community.
Respiratory services are available to people (18 years or over) and have a confirmed respiratory condition, such as:
- Chronic obstructive pulmonary disease
- Bronchiectasis (not cystic fibrosis)
- Interstitial lung disease
- Asthma
Where are appointments available?
Appointments are offered in Osborne Park as well as online (telehealth).
Pricing
For appointments with the Pulmonary Physiotherapist, we charge $98 for an initial assessment and $80 for review appointments. Clients referred by their GP under a Team Care Arrangement will be eligible for a Medicare rebate. Concession card holders will be bulk billed.
Referral process
Referrals are accepted from GPs and Specialists only for this program.
For General Practitioners
Referrals to our services are simple; just download and complete the referral form. Please ensure that you attach a copy of the GP Management Plan and Team Care Arrangement. Clients will be asked to pay the full cost of the consultation and a Medicare rebate will be refunded into their bank account via HICAPS if applicable.
