About the Program
Living Well is designed for people with one or more chronic conditions. It has been shown to lead to significant improvements in symptom management, self-reported general health, fatigue and disability. The Program aims to help people with self-management of their condition.
To be eligible for referral, clients need to have a long term chronic condition such as:
- Respiratory disorder
- Heart disease/ stroke
Typical Program Activities:
Free workshops are run over 2.5 hours, once a week, for six weeks at community locations. The workshops are conducted by trained leaders in a friendly and supportive environment. Participants learn ways to self-manage their condition and gain practical skills and strategies to help them cope with problems.
Topics covered include:
- Symptom management
- Pain management
- Exercises to maintain and improve strength and flexibility
