All our chronic disease programs have been designed to help prevent and manage chronic diseases in our community through the promotion of healthy lifestyle behaviours, with the aim of reducing hospital admissions and the burden of chronic diseases in our region. They are all community based and easily accessible.

The Health and Wellness Program offers people at risk of heart disease or type 2 diabetes access to a free 6-week group program run by a team of diabetes educators, dietitians and physiotherapists.
